Arizona Department of Health Services (AZDHS)
The AZDHS regulates the operation of Sober Living Facilities within the State of Arizona. They accept complaints related to quality of life and quality of care, including residents’ rights, abuse, neglect, staffing and environmental concerns.
To report a residence that you may suspect is running an unlicensed sober living facility within the community, you can use the AZDHS online portal.
